Documents necessary to lodge an application to resume Australian Citizenship:

 
□ Form 132 completed, signed, dated

□ 1 passport photograph – endorsed on the back by the authorised person (please read the instructions on the form 132, pages 1 and 2). The endorsement should be inscribed as “This is a true photograph of (applicant’s name)” with the signature of the authorised person. Please also ensure that the “proof of identity” section in form 132 has been completed.

□ Valid ID document with photo, signature and address

□ Evidence of former possession of Australian citizenship
 full birth certificate (if born in Australia) OR
 original Evidence of Australian Citizenship

□ Evidence of change of name (if applicable)
 Australian marriage certificate OR
 deed poll issued by the Australian Registrar of births, deaths & marriages OR
 Statuary Declaration and at least 2 documents evidencing usage of new name

□ Evidence of acquisition of a foreign citizenship
 citizenship certificate and
 a ruling from the relevant authority where you acquired your other citizenship. This statement should provide the date and relevant article of the Citizenship Law under which you acquired your other citizenship. Please also provide a certified English translation.

□ Evidence of character
 police checks for applicants aged 18 and over from all countries where applicant has resided in the past 10 years (except Australia) together with the official English translation

□ Fee (please see attached instructions on how to pay the fee). For the correct fee amount, please contact our office. 
Additional postage costs for return of documents by registered mail in the value of 15.00 HRK or 5.00 EURO.
