Documents necessary to lodge an application for conferral of Australian Citizenship (children under 18):

□ Form 1290 completed, signed and dated
Proof of Identity declaration (question 22) completed for all applicants. For detailed instructions please refer to Form 1290i.

□ Endorsed passport photo for applicant
For detailed instructions please refer to Form 1290i.

□ Original birth certificate & English translation for applicant

□ Passport or other travel documents for applicant

□ Valid ID document for responsible parent (if child under 16 years)

□ Other forms of identification (with photo, signature and correct address)

□ Evidence of former possession of Australian citizenship for parent
Full birth certificate (if born in Australia) OR
Original Evidence of Australian Citizenship

□ Evidence of acquisition of a foreign citizenship for parent
A ruling from the relevant authority where you acquired your other citizenship. This statement should provide the date and relevant article of the Citizenship Law under which you acquired your other citizenship. Please also provide the English translation of the certificate from the Court interpreter

□ Fee (Please see attached instructions on how to pay the fee). For the correct fee amount, please contact our office.
An additional return postage cost for registered mail in the value of 15.00 HRK or 5.00 Euro. (Please see attached instructions on how to pay the fee!)
