Documents necessary to lodge an application for registration of Australian citizenship by descent

□ Form 118 completed, signed and dated.
Question 31 Proof of Identity needs to be completed. Please refer to form 118 for instruction.

□ Endorsed passport photo for applicant
Photo must not be more than 6 months old. Please refer to form 118 for instruction.

□ Original birth certificate for your child/children

□ Valid ID document with photo, signature and address for responsible parent of minor child
□ Valid ID document with photo, signature and address for child

□ Evidence that at least one parent is an Australian citizen at the time the child was born Australian full birth certificate if born in Australia or Australian citizenship certificate

□ If you are a citizen of another country
For example Croatia or Bosnia, you must provide evidence of having obtained that citizenship from the relevant authority (eg 'rješenje'). This document should state the date and exact article of the Citizenship Law under which you acquired the citizenship

□ Evidence of Character for applicant
Police clearance certificate for all applicants aged 18 and over from all countries where applicant has resided for 12 months or more. Please provide the original certificates together with the certified English translations.
Croatia - Ministry of Justice and the Municipal Court.
Bosnia & Herzegovina – Ministry of Interior and Municipal Court

□ Evidence of change of name
Australian marriage certificate or deed poll issued by the Registrar of births, deaths & marriages

□ Fee - please refer to the Australian High Commission in London for instructions on how to pay the application charge. 
 

ALL DOCUMENTS MUST BE ORIGINALS OR CERTIFIED COPIES OF THE ORIGINAL. ENGLISH TRANSLATIONS ARE REQUIRED FOR ALL DOCUMENTS THAT ARE NOT IN ENGLISH
